With support from United Way of the Wine Country and the Center for
Well-Being, Hearts of Sonoma County launched a multi-faceted outreach
and media campaign on August 1. The “It’s Up to Us” media campaign aims to raise awareness of the importance of blood pressure control for heart health.
It’s Up To Us is about empowering the communities that heart
disease impacts the most. It brings us together, because there are too
many heart related deaths that shouldn’t happen. It provides the
information and raises awareness, driving people to join the cause and
help their friends, family, and neighbors. This is an issue that affects
all of us, it’s time we fight back, together.
Through the media campaign, Sonoma County residents are encouraged to
visit CheckYourBP.org or TomeselaPresion.org to learn more. This
campaign will be strengthened through direct community engagement by the
Center for Well-Being’s Promotores de Salud. The Promotores will be
focused on helping residents check their blood pressure numbers and
equipping them with tools to access critical self-care resources.
“It’s Up To Us” is a key component of the Hearts of Sonoma County
initiative to reduce deaths and disability from heart attacks and
strokes in Sonoma County through early detection and better management
of high blood pressure and other cardiovascular disease risk factors.
