    Objective: Inform CHI members and partners about current COVID case and vaccine rates and County-community planning for pediatric vaccines and boosters

  3. CARES Act Community Health Worker Grant Project

    Presenter: Kim Caldewey, Public Health Section Manager, Sonoma County Department of Health Services
    Objective: Inform CHI members about a project to enhance and expand the community health worker (CHW) workforce in Sonoma County as part of a national program to support COVID-19 response and prevention in communities at high risk and most impacted by the pandemic.

  5. Health Action Transition Progress Report
