3. Community Health Workers: Framing the Opportunity

(Presentation)

Facilitators/Presenters: Karissa Moreno, Executive Director, Center for Well-Being and Cathryn Couch, CEO, Ceres Community Project

Objective: Provide an overview of who community health workers (CHW) are, the range of roles they can play in preventing and treating cardiovascular disease (including upstream SDOH work), how they can be licensed and reimbursed, and the past and current work that Hearts of Sonoma County and member agencies have been doing to support CHW workforce development and integration into care teams. Understand the opportunities to build on work that emerged during COVID as well as new opportunities under CalAIM.

4. CDC Funded Community Health Worker Project (Presentation)

Time: 8:55 AM-9:05 AM

Facilitators/Presenters: Kim Caldewey, Public Health Section Manager, Sonoma County Department of Public Health

Objective: Learn about the new CDC funded grant that the county received to support the development of a CHW workforce. Explore which Hearts members are participating in this project and what is the opportunity to advance cardiovascular health.

5. Community Health Worker Discussion

Time: 9:05 – 9:25 AM

Facilitators/Presenters: Karissa Moreno, Executive Director, Center for Well Being

Objective: Assess which Hearts members are currently exploring opportunities to develop a CHW workforce, discuss how Hearts members are participating in CHW grant projects (CDC grant/ Sonoma Connect/ First 5/ CHW Clinical Integration Pilot), and explore how Hearts and Hearts members can leverage these grant-funded opportunities to be poised to transition to advance payment reform via CalAIM to strengthen integration of CHWs in order to improve cardiovascular health in Sonoma County.
