2. Population Health Management at Partnership Healthplan

  • Objective: Learn about the goals, strategies and resources of Partnership Healthplan of California’s new Population Health Department, and how this team can collaborate with and support the goals and work of Hearts of Sonoma County.

3. PHASE & Healthy Hearts at Petaluma Health Center

Facilitators: Jessica Moore, Petaluma Health Center (PHC)

  • Objective: Petaluma Health Center was an early adopter of the Kaiser PHASE (Preventing Heart Attacks and Strokes Every Day) protocols. Jessica will discuss PHC's expanded team-based approach as well as the data and analytics that supports their program for improving cardiovascular health.
