2. West County Health Center CHW Pilots

Objective: Learn about West County Health Center's new CarePlus branch as the umbrella for all enabling services offered by CHWs to our patients. CHW services include patient navigation, health coaching and behavioral health services to high-risk communities, linking patients to resources both within the clinic and the broader community.
