3. Cardiovascular Disease Trends in Sonoma County

Objective: DHS Research & Epidemiology will share publicly available sources of data for monitoring cardiovascular health and review trends over the last 10 years for Sonoma County.

4. Expanding Data Collection & Reporting to include Upstream Measures

Objective: We’ve recently identified five priority areas for our work that relate to health equity, moving the work towards upstream and prevention activities, and addressing health-related social needs. Given these focus areas, what additional data might we track and report on to measure our effectiveness?
