2. Clinical Tobacco Update

Objective: Revisit the survey the Tobacco team conducted a year ago to assess clinical practices around tobacco screening and referrals and share how several clinical teams have made changes and improvements.

3. Social Isolation, Stress & Depression – How to Best Support Patients

Objective: Stress, social isolation and depression all have negative impacts on health. The pandemic and fires have exacerbated these conditions for many, and now as the holidays and shortest days of the year are approaching. What are Hearts of Sonoma County members and partners doing to support patients’ and clients’ mental and emotional health? How might Hearts of Sonoma County work together to address these impacts as known risk factors for cardiovascular disease?
