3. Follow-Up Discussion from September Meeting: Self-Monitoring Blood Pressure

Objective: Revisit and discuss data from Carium’s self-monitoring blood pressure pilots. Hear about Santa Rosa Community Health’s strategies to support blood pressure self-monitoring. Discuss other Hearts of Sonoma County member organizations’ efforts to support patients in adopting blood pressure self-monitoring, including challenges and lessons learned.

4. Clinical Tobacco Cessation Project Reports

Jewish Community Free Clinic: Dr. Deborah Roberts, Clinic Director and Zhanna Ghukas, Clinical Project Specialist

Objective: Share progress in two community health centers implementing clinical tobacco cessation projects, including strategies implemented, challenges encountered, and lessons learned.
