3. Heart Safe Communities

Facilitators/Presenters: Ramona Faith, Petaluma Health Care District

Objective: Learn about Heart Safe Communities and the work that Petaluma Health Care District has done to make Petaluma “Heart Safe”. Discuss how Hearts of Sonoma County can work with the Heart Safe initiative to leverage this work across Sonoma County.

4. Integrating CHWs into Care Teams: Lessons Learned & Next Steps

Time: 8:45 AM-9:10 AM

Facilitators/Presenters: Karissa Moreno, Center for Well Being and Hearts Leadership Team

Objective: Get updated on efforts to integrate community health workers into care teams, including lessons learned. Discuss opportunities to build on this work and to leverage CalAIM to support further efforts.

5. Self-Monitoring Blood Pressure

Objective: Get an update on what Carium is learning about self-monitoring blood pressure efforts from their pilots around the country.
