3. Portrait of Sonoma Discussion

Facilitator: Karissa Moreno, Center for Well Being, and Lori Houston, Department of Health Services

Objective: Continue the discussion begun in March with the presentation about the Portrait of Sonoma County 2021 Update. Discuss what the data reveals about disparities in outcomes and explore the relevance of this data for cardiovascular risk and disease. Discuss implications for Hearts’ Portfolio of Interventions, including additional areas of focus for policy and system change work.

4. RCHC PHASE Cohort – Final Evaluation & Next Steps

Presenter: Michelle Rosaschi, Redwood Community Health Coalition

Objective: Understand how the PHASE collaborative funded by Kaiser Permanente has impacted the work community health centers are doing to prevent and treat cardiovascular disease and the medical risk factors for CVD. Discuss the implications of this learning for Hearts’ ongoing work.
