2. Health Action Transition and What’s Next for Committee for Health Care Improvement & Hearts of Sonoma County

Presenters: Cathryn Couch, Ceres Community Project; Lori Houston, Department of Health Services; and Jason Cunningham, West County Health Centers, CHI Executive Team

Objective: Brief Hearts’ members on the transition of Health Action from being supported by the county to an independent organization. Understand the vision for Health Action going forward and why Committee on Health Care Improvement (CHI) and Hearts will no longer be a formal part of Health Action. Understand the process to develop a new vision and plan for CHI and Hearts’ work and how Hearts members will be able to participate in that process. Get updated on the plan for supporting Hearts during this transition.

4. Clinical Tobacco Cessation Project Reports

Presenter: Sonoma County Indian Health Project: Diana Zamora RN, BSN, SCIHP PH Nurse Manager

Objective: Share progress in one community health centers implementing clinical tobacco cessation projects, including strategies implemented, challenges encountered, and lessons learned.
