Objective: Provide an overview of Partnership HealthPlan’s goals for the implementa-tion of Enhanced Care Management and Community Supports in Sonoma County, in-cluding target populations and goals for each Community Supports intervention. En-gage in a discussion about how Hearts members (health care and CBOs) can use these new benefits to support cardiovascular health and better disease management.
