2. Health Action Redesign - Stakeholder Engagement

Time: 8:05 – 8:45 a.m.

Objective: Collaborative Consulting will share the status of the work of the Health Action Transition Team and solicit feedback from Council members on overarching vision and goals for the future of Health Action.

3. Latino Family Educational Summit

Time: 8:45 - 8:50 a.m.

Presenters: Magali Telles, Community Engagement Director, City of Santa Rosa

Betzy Chavez, Education Specialist, Hanna Institute

Objective: Provide an overview of the upcoming Latino Family Educational Summit and encourage the Health Action network to share this opportunity.

4. Sonoma Connect: A Vision for Sonoma County

Time: 8:50 - 9:20 a.m.

Presenters: Sarah Rock, Rock Results

Dan Schurman, Community Health Investment Manager, Providence Sonoma County

Cynthia King, Director of Programs, Redwood Community Health Coalition

Objective: Share the overall picture of the ACEs Aware grant in Sonoma County including connecting to Health Action Chapters, closed loop referral systems, working with clinics, and how Sonoma Connect will help support health in Sonoma County.

5. Sonoma County Vaccine Update

Time: 9:20 - 9:25 a.m.

Presenter: Dr. Urmila Shende, COVID-19 Vaccine Chief, Sonoma County Department of Health Services

Objective: Brief Council members on latest Sonoma County COVID case/vaccine data and information.
