My Care, My Plan: Speak Up, Sonoma County Steering Committee

The Committee for Healthcare Improvement launched My Care, My Plan: Speak Up, Sonoma County as a community-based initiative to educate and motivate the public about advance care planning. The initiative was active  2014-2019. See the timeline of activities

MCMP was lead by a collaborative of organizations and individuals from the private, public, nonprofit and volunteer sectors, including local health care and social service organizations and other community partners who served on the Steering Committee during the initiative's five years in operation.
