Create and Communicate Your Advance Care Plan
Advance care planning is for every person in Sonoma County who is age 18 and older. Advance Care Planning is a thoughtful process you carry out before a health crisis occurs or you become unable to speak for yourself. This involves exploring, communicating about, and documenting your wishes for the type of future medical care and treatment you would want. Advance care planning leads to completing an Advance Health Care Directive.
The Committee for Healthcare Improvement launched My Care, My Plan: Speak Up, Sonoma County as a community-based initiative to educate the public about advance care planning to motivate people to complete Advance Health Care Directives. The MCMP initiative was active 2014-2019, offering free community workshops and presentations to community groups regularly. (See timeline of MCMP activities and accomplishments.)
Although the local initiative is no longer active, advance care planning is more critical than ever. Resources and answers to frequently asked questions continue to be available on this web site to support you in your own advance care planning journey.
